FAQ's
1. How do I apply for a Prudentialife
plan online?
These are the steps in applying for any of our
pre-need plans over the web:
a. Register
your Username, Password and other Personal Details - Before you can purchase any of our pre-need plans online, we require you to register your username and password. We also ask for your personal details such as your birth date, email address, etc. You can activate your username and password through the link in the email we sent to the email address indicated in the Registration Form. After clicking on the link for activation, you can log-on to our Get A Quote facility.
b. Determine
the Product Type you want to purchase - We recommend that you read through
our product literature to determine which plan/s
suit/s best your needs. Our product literature
pages contain information on each plan's features
and benefits. There are also links to these
pages in the Get A Quote Facility page just
in case you went to it without going through
the products' features and benefits.
c. Calculate
for monthly/quarterly premiums - Our Get A Quote facility allows you to calculate
your payments to Prudentialife based on the
type of plan, payment term and payment mode
you choose.
d. Fill out online
application forms - Our Plan Application
(PA) form contain applicable fields for the particular Plan.
Majority of the information is required. You need to input
personal information as well as information on your Beneficiaries,
Co-Planholders and Scholar (for Education Plans). We require
a valid Philippine mailing address of the Planholder, even
if Planholder works outside the country or even if billing
will be done electronically.
e. Print the filled
out forms - Print or save the filled-out
PA form for your reference. The signed PA form will only be
required on the following:
e.1 For applications to be paid via online credit card payment
option in which initial installment is more than Php500,000.00
together with a photocopy of two valid IDs.
e.2 For applications to be paid via remittance option, regardless
of the amount of initial installment. Photocopy of two valid
IDs is also required.
For applications requiring signed PA form, the documents will
be sent to Prudentialife through mail/courier. Mailing instuctions
are included in the email notification sent by the system.
Processing of the application commences as soon as we receive
the electronically filled-out PA and online payment. The Plan
Contract for the application that requires signed PA and photocopy
of two valid IDs will only be released upon receipt of the
documents. For Plan/s with Co-Planholder, the Co-Planholder
Declaration Form is required to be printed, signed by the
Plan Applicant and the Co-Planholder and sent to Prudentialife.
We will notify the Plan Applicant via email once the Plan
Contract is mailed to the indicated Philippine mailing address.
f. Payment
- There are two options for paying the plan, either
via online credit card payment or via remittance. For online
credit card payment, you will be directed to our payment page
that is also secured via SSL (This means transfer of electronic
details is encrypted). In the payment page, you will provide
credit card information. Once approved, the initial payment
indicated in the PA form will be deducted from your credit
card. You may also opt to enroll in our Auto-Charge Arrangement
(ACA). (see section on Billing
for details on the ACA). The online transaction
details and notification you will receive via e-mail will
serve as proof of initial payment. We advise you print this
for your own reference. Your Official Receipt will be sent
to your mailing address, together with your Plan Contract.

Who are the people involved in the Plan?
Planholder - The owner of the plan who
must be of legal age
Beneficiary - The person designated by
the Planholder to receive the benefits of the
Plan in case of his death
Scholar - For Education Plans. The person
designated by the Planholder to use the Plan;
normally, 0 to 10 years old; or Grades I to
V (if the child is already schooling, disregard
the age)
Co-Planholder - The Planholder's co-insured
who may be his insurable spouse, child, parent,
brother or sister. This is applicable for premium
plans only and Powered Omni plan.

Are all Plan Types available for purchase online?
Yes, all Plan types are sold online.
4. How will I know
if my Plan Application is approved or pended?
We check and verify the PA form electronically. Verification
for signature however is done as soon as we receive the signed
PA form (Applications with Remittance payment mode and Credit
Card Payments with an initial installment greater than PhP
500,000.00) through courier/mail. Prudentialife will send
you an email informing you of your Plan Application status.
We indicate the cause/causes for disapproving or for the pending
of your Plan Application and we allow you to correct any missing
or wrong information within 30 days. Non-compliance may mean
forfeiture of your initial payment. For approved Plan Application,
your Plan Contract will be sent to your billing address via
courier/mail.
5. What are the
causes for pended and disapproved Plan Application?
a. For applications paid online via Credit Card, the Cardholder
is not the Planholder or Plan Applicant.
b. Failure to send the signed PA Form to Prudentialife. Plan
application will be accepted but without insurance rider.*
c . PA without Plan Applicant's signature. Plan application
will be accepted but without applicable insurance rider.*
d . PA "signed by" or "signed for" another
person other than Plan Applicant/Planholder. Plan application
will be accepted but without applicable insurance rider.*
e . Signature/s is in print, block letters, non-English readable
and thumb mark. Plan application will be accepted but without
applicable insurance rider.*
f . Discrepancy in Plan Applicant's name and signature
g . Fictitious mailing address Billing Address and/or Business
Address.
* For applications requiring signed PA
6. How will I receive
my Plan Contract and how long?
Prudentialife will process and issue your Plan Contract within
seven (7) to ten (10) working days from the date of receipt
of the electronically filled-out Plan Application and payment.
The Plan Contract for the application that requires signed
PA and photocopy of two valid IDs will only be released upon
receipt of the documents. Plan Contract will be sent to your
mailing address. You will also receive an advise via email
on the progress of your application and once the Plan Contract
has been sent via mail/courier.
7.
Do I still get the same plan benefits and features
for online purchases as those of offline purchases?
Plans purchased online carry the same benefits
as those sold or bought through our Sales Associates.

8.
Why do I have to register my username, password and personal details before being allowed to purchase a plan?
The registration process has been required so we may verify the identity of the person/s trying to purchase a plan and the validity of the email address/home address indicated in the registration form. We send out e-Newsletter for all our registered users, with the option to opt-out or request to remove your email address from our Mailing List.
9. What if I forgot my username or password?
Just log-on
to "forgot
username and password page" and indicate
the e-mail address you used for the registration.
We'll then send to your e-mail address your
username and password. |
