CLAIMS AND PROCEDURES

REMINDERS TO OUR AVAILING PLANHOLDERS:

We would like to make your claims as convenient as possible. Just go through this page thoroughly and we assure you that your claims will be a breeze.

To make claims even more convenient, you can actually fax (894-5353) or send a scanned copy of your documents as attachment in your email before visiting our office. This way, we will still follow the cut-off schedules but you will only come to our office once to show original documents and claim your check.

Required Documents:

Original and photocopy of:

  • Plan Contract
  • Certificate of Full Payment
  • Authorization Letter, if check is to be secured by a person other than the Planholder/Scholar
  • Special Power of Attorney, if desired payee is other than the Planholder or Scholar
  • Identification card of Planholder or Scholar

    Additional Requirements for Heritage Plans
  • Assessment/Registration Form with breakdown of misc. and other fees with no. of subjects and units taken *
  • Official Receipt (in case planholder has already paid for the tuition fee) *
  • Course Curriculum (if scholar is Freshman) *

 Procedures in Availing Education Benefits:

I. Over-the-Counter Procedure:

  1. Secure a number and SOD Control slip from the School Operations Dept. (SOD) Counter.
  2. Fill out the Control Slip and wait for your number to be called.
  3. When the Receiving Staff calls your number, go to the SOD counter and submit all the required documents to the Receiving Staff.
  4. Receiving Staff shall check the completeness of your documents and will advise you of lacking documents, if any.
  5. If documents are complete, you may secure your check subject to the cut-off schedule as follows:

    III. Direct to School Payment Graduating or last availment (with Assessment Form only)

    3:01 - 5:00 PM
    8:00 - 10:00 AM (following day)
    10:01 - 12:00 PM (following day)
    12:01 - 3:00 PM (following day)

  6. For Legacy or Peplife/Powered Plans, the check is always payable to the Planholder or to any designated representative as specified in a Special Power of Attorney (SPOA).

Checks Encashment:

You may encash your checks with RCBC Gamboa Branch near our office. Be sure you have any of the following identification documents to be able to encash your checks:
