INSURANCE REQUIREMENTS

Insurance Benefits are given to the Planholder upon meeting the following Insurance Requirements:

• 18 - 65 years old at the time of purchase or reinstatement
• In good health at the time of purchase or reinstatement
• One year contestability period from the date of purchase or reinstatement
• Payments must be up-to-date or fully paid
