These are the set of questions on change
in plan data, plan transfer and availing
Planholder loans frequently asked by our
Planholders. For answers to question on
claiming/availing plan benefits and reinstatement

REQUESTING FOR CHANGE IN PLAN DATA
1.
Can I change my designated beneficiary/scholar?
Yes. You just have to fill out and sign
a Request for Change in Plan Data form.
This form is available at our Head Office
in Gamboa, Makati and at all Prudentialife
branches. We also encourage you to utilize
the Online RCPD form that is available
at our Planholder
E-Services Page.
Additional Note
for Change in Scholar
- The new scholar must have the same
age as the previously designated scholar
or is younger. (If younger, age difference
should not be more than 5 years).
- If change is done during availment,
only the remaining coverage/benefit
unavailed of by the original Scholar
can be enjoyed by the New Scholar.

Are there documents necessary to effect
any change of information in my Policy
Contract?
Some documents maybe required aside
from the RCPD form and it will depend
on the information you wish to change.
Please refer below for type of RCPD-General
Information and the corresponding document
requirement.
a. Change in Scholar - Photocopy of
Scholar's Birth Certificate
b. Change in Date of Birth of Planholder/Scholar

Where can I go or who can I see if I
want to change some information in my
Policy Contract?
Any change in plan data must be coordinated
with the Customer Service Department.
You may call Prudentialife Direct at
864-8000 for inquiries on procedure
and requirements for this purpose. You
may also utilize our Online RCPD form
that is available at our Planholder
E-Services Page.
4.
Can I ask a representative to fill out
and sign the RCPD form on my behalf?
No. Only the planholder should sign
the RCPD form and all RCPD-related transactions
must be made by the planholder himself.
The specimen signature must also match
the one found on your plan application.
5.
Can I just relay any change I want done
in my policy by phone?
No. For our mutual protection, a written
request from you is required in order
to change any data in your policy.
6.
My specimen signature changed along
with the change in my civil status.
Is there a need to change my specimen
signature in the policy?
No. There is no need to change the signature
in your policy. Just fill out and sign
an RCPD form and attach a copy of your
marriage contract. The RCPD specimen
signature should reflect your new signature
and the corresponding change in your
civil status.
6.
In the absence of an RCPD form, can
I just write a letter to request for
a change in my billing address?
Yes. A letter requesting for a change in billing
address is acceptable. You may mail
your letter to our Customer Service
Department at the address stated above
or fax it at 894-5353. You may also
change your billing address online.

ON
TRANSFERRING OWNERSHIP OF A PLAN
1.
What are the requirements to transfer
a plan?
a. The plan must
have up-to-date status.
b. You should surrender the following
documents:
- Original Policy
Contract
- Original Certificate
of Full Payment (for fully paid plans).

Original Master List of School and Certificate
of Scholarship (for heritage education
plans).
c. Accomplished and signed Request for
Change in Plan Data (RCPD) form*
d. Both the transferor (you) and your
transferee should fill out and sign
Plan Transfer form*
(to be notarized).
For plans with co-planholder, co-planholder
must also sign the transfer form
e. The Transferee must fill out, sign
and submit a new Plan Application**.
g. The transferee must be a resident
of the Philippines and must reside within
Prudentialife's marketing area (for
life plan).
h. Payment of Transfer fee (PhP100.00)

Note:
Transfer of plan ownership is subject
to the approval of Prudentialife.
2. What are
the insurance requirements on the new
owner?
The new owner/planholder
will be subjected to the following insurance
requirements:
a. He should be 18 to 65 years old at
the time of transfer.
b. He must be in good health at the
time of transfer.
c. He must undergo one-year contestability
period.
Note: The
transferee/new planholder will enjoy
a coverage equivalent to the remaining
insurable age of the transferor.
3.
Can a plan be transferred during the
paying period?
Yes, a plan may be transferred during
the paying period provided all the requirements
to effect the transfer are complete.
All privileges and obligations that
go with the plan are transferred to
the new owner/transferee. The remaining
installments (if any) shall therefore
be the liability of the new owner/transferee.
4. If I am
abroad, can I assign a representative
to conduct the transfer of my plan on
my behalf?
Yes. However, you will have to be the
one to sign on the Transfer Form and
must execute a Special Power of Attorney
(SPA) indicating the name of your assigned
representative and the specific authority
granted to him. You may then advise
your assigned representative and/or
the Transferee to coordinate with our
office regarding facilitation of the
plan transfer.
5. Is it possible
to transfer the plan to a non-relative?
Yes. You may transfer your plan to any
person, whether he/she is related to
you or not.
ON
AVAILING PLANHOLDER LOANS
1. When
can I avail planholder loan?
For Life Plan
and Education Plan
Loan can be availed upon full payment
of plan and receipt of Certificate of
Full Payment (Life and Education Planholder
loans are facilitated by our affiliate
company PLPI Finance, Inc. / PFI).
For Pension
Plan
Loan can be availed upon payment of at
least 40% of the contract price. Status
of plan must be up-to-date (no 60 days
grace period) at the time of loan availment.
Plans reinstated by redating or updating
can only avail of the loan benefit after
a year from date of reinstatement. Existing
loan must first be fully-paid before loan
renewal. Loans in other pension plans
must be fully-paid or up-to-date.
2. How much loan can I avail
and what are the payment terms?
For Life
Plan and Education Plan
Loanable amount depends on the category
of the plan. Payment term is minimum of
6 months and maximum of two (2) years.
Loan is paid monthly either with Cash
or PDC’s.
For Pension
Plan
Loanable amount is equivalent to 70% of
the Annual Cash Value of the plan corresponding
to the Pension Plan anniversary date attained
prior to the application of the loan.
The loan shall be payable within one year
in required no. of installments, depending
on the chosen mode of payment - annually,
semi-annual, quarterly, or monthly.
3.
What are the requirements in availing
a Planholder loan?
Below
are the requirements in availing Life and
Education plan loan:
- Original Certificate of Full Payment
- Original Policy Contract
- Residence Certificate
- Valid Identification Card
- Post-dated checks (for check payments)
Below are the requirements in availing
Pension plan loan
- Original Certificate of Full Payment
(if fully-paid)
- Original Policy Contract
- Loan Application* (3 copies)
- Credit Agreement* (5 copies)
- Residence Certificate
- Valid Identification Card

4. Where
can I proceed to avail a Planholder loan?
For Life Plan and
Education Plan
You may visit any PLPI Finance, Inc. (formerly
PCC). Click here to view list of PFI branches.
For Pension Plan
You may visit any Prudentialife branch
office nearest you or to the Pension Loans
and Benefits Department (PBD) at the PLPI
Head Office, 3rd Floor, 118 Gamboa Street,
Legaspi Village, Makati City. You may
also file loan application and pertinent
documents to any PFI Branch. PFI will
only act as application center for loan
processing and release. All loan documents
and records will still be forwarded to
PBD at PLPI Head Office. A processing
fee of PhP200.00 or 3% of the loanable
amount will be charged for Pension loans
applied through PFI.
4.
Can I assign a representative to apply
for a loan on my behalf?
For Life Plan and
Education Plan
No. You must transact personally to avail
a loan.
For Pension Plan
Yes. However, you should be one to sign
the Loan Application, Promissory Note
and Highlights of Promissory Note. You
must also execute a notarized Special
Power of Attorney indicating the name
of your representative and specific authority
granted to him.
